After Coastline receives this application, an email will be sent to you with a link to our Student Information Release Authorization Agreement form. Please print, read and sign this document and submit back to Coastline.

This application is considered a legal document and will become a permanent part of your record. Falsification of this document may be cause for dismissal and other legal action as deemed appropriate. I certify that all information provided is correct and I understand that I am being conditionally accepted by the institution(s) associated with my chosen degree pathway.
